Invisalign is a trademarked name for a proprietary method of orthodontic therapy, developed by Align Technology, Inc. It is promoted as an “invisible” way of straightening teeth without using traditional braces, instead using a series of custom fabricated aligners designed using 3-D technology to gradually and sequentially move teeth to their desired positions. The principle behind the orthodontic tooth movement is based on Kesling’s setup using the positioner.

The product initially met with resistance from many orthodontists who criticized the manufacturer for rushing the product into production, and concentrating too heavily on marketing at the expense of product quality. The company claims that preliminary issues have largely been remedied in the last several years. Now over 70% of all U.S. orthodontists are certified to treat patients using Invisalign.

The primary advantage of Invisalign is cosmetic; the aligners are difficult to detect, rather than invisible per se, and therefore has become particularly popular among adults who want to straighten their teeth without the unflattering look of traditional metal braces, commonly worn by children and adolescents, and therefore carrying the connotation of immaturity.

Another advantage over traditional braces is that the aligners can easily be removed for eating, and brushing teeth. The treatment typically requires fewer visits to an orthodontist, as well. However, Invisalign treatment is typically more costly than traditional braces, although the cost of treatment varies considerably.

Invisalign is not as effective as traditional fixed orthodontic appliances, and is unable to correct many orthodontic problems such as severe crowding, openbites, overbites, or underbites.

Treatment time can be longer using the Invisalign method rather than coventional orthodontic appliances, and the practitioner’s ability to fine-tune the final result may be compromised by the limitations of its technology. In addition, being removable, the aligners are at risk of being lost or damaged, and they must be worn constantly to avoid delaying the treatment process. Because of this potential, many orthodontists never use this treatment for children or teenagers whose second molars (12-year molars) have not erupted.

These clear treatment retainers are most suitable for skeletal and dental Class I, mildly crowded orthodontic cases with good profile that normally can be treated with removable retainers. Compliance with these appliances is necessary, and the patient is expected to wear them for at least 20 hours per day.

Unlike traditional dental appliances, though, most patients are actually motivated to wear these appliances because of their superior esthetics. There is an advantage for patients using these appliances if they have periodontal or gum problems because, unlike appliances like braces, these removable appliances allow easy daily oral hygiene practices.

Extraction treatment using these appliances must be cautioned because root parallelism in the extraction sites are usually compromised and the periodontal outcome in the extraction site needs to be further studied. Certain types of tooth movements are also limited with these appliances, such as rotations of premolars and intrusion of incisors.

Generally, orthodontists can perform more complex movements and can supplement these clear retainers with a brief period of fixed appliances (traditional braces) to either attain complex tooth movements prior to starting or detailing the dentition at the conclusion of treatment. OrthoClear is a company that produces a rival brand of clear retainers for remedial tooth arrangement, established in 2005.

Orthoclear: the Betamax of Transparent Tooth Straighteners?

In the late 1970s and early 1980s, a format war was fought for control of the home video recorder market. Betamax, introduced by Sony, was the first commercially popular technology, and, in the early days, was a superior technology to the upstart VHS, developed by JVC. The Betamax tapes were smaller, allowing for a better camcorder technology, and they had higher video and audio quality. The main drawback to Betamax was the length of the tape, which at first was only 1 hour, compared to the 2-hour VHS tapes. With longer tapes and cheaper equipment, the VHS tape eventually won the market, despite the initial superiority of the Betamax technology.



Although Invisalign was the first clear aligner tray utilized for tooth straightening, several employees of the Align company that produced the system, broke away to develop a competing system: OrthoClear. Although many dentists regarded the OrthoClear system as superior, a lengthy legal battle led to the demise of OrthoClear and the dominance of the Invisalign brand.



Bad Blood



Align Technology was founded in 1997 by Zia Chishti and Kelsey Wirth, and received approval from the Food and Drug Administration to market the Invisalign system the following year. Both founders left the company by 2003. Chishti then founded OrthoClear in 2005 to produce a competing design for utilizing clear plastic braces to align teeth.



However, the founding of OrthoClear was immediately accompanied by lawsuits from Align, which alleged patent infringement and illegal conduct by Chishti and others. In the complaint, Chishti is named as having met with patent lawyers, software engineers, and other employees in order to recruit them for his new enterprise, all in violation with codes of conduct and his contractual obligations, which he sought to avoid.



According to OrthoClear, the lawsuits filed by Align were meritless, since Chishti was the chief inventor named on the Invisalign patents. Align, they say, was a company that had lost 90 % of its original employees and had become a company without innovative ability to extend the scope of its practice, and that the people had left the company partly to escape from the profiteering attitude of the forces that had taken hold of the company.



A Superior Product?



During the short period of time that OrthoClear aligners were being produced, they developed an impressive reputation among dentists. Dentists cited the ability of OrthoClear systems to correct misalignments that Invisalign could not. Also, they said that although the Invisalign software was “slicker,” the OrthoClear mouthpieces fit better. In addition, doctors pointed out that many times Invisalign treatments did not work as planned and patients were required to undergo additional treatments for as long as 6 months after the planned completion of treatment, whereas this was never the case for OrthoClear. In addition, the OrthoClear treatments were cheaper.



The Demise of OrthoClear



However, in the end, the legal battle would doom the new company. After fighting a lawsuit for its entire existence, the company agreed in September 2006 to close its doors and turn all its intellectual materials over to Align in exchange for $20 million.



Although the company producing the superior product was destroyed, the transfer of intellectual property back to Align meant that the older company could incorporate technological improvements into its design, resulting, hopefully, in an improved version of the Invisalign process.

It used to be that we didn’t have a choice when it came to braces. Well maybe a choice in the rubber bands, but as far as the “metal mouth” went we really couldn’t choose to make it look less obvious. But lately they have come out with something that’s called Invisalign. I’m assuming most people know what this is because it has been out for a few years, but just in case some people don’t – the name says it all. These are essentially “invisible” braces.

You can’t actually tell if a person is wearing these or not unless you get really close and even then it’s iffy! This is a terrific choice for adults that want to get their teeth fixed and straightened out, but they really don’t want to deal with the original look of braces and nowadays you see a lot more people wearing the Invisalign rather than the metal brace options. I guess it also depends on you as well.

Regular braces are rather painful at the beginning, plus you can also expect things like not being able to eat certain foods, and experience gums bleeding for a few days until they get used to the wires and sensitivity. However painful, braces also are going to stay on for a specific amount of time. Usually anywhere from 5 weeks on up to a few years.

Whereas the Invisalign is supposed to be less painful to wear and “put on”, but you also have to take them off every 2 weeks.
